Background and Purpose: Maintaining a healthy lifestyle is essential in preventing and managing chronic illnesses. Persian Medicine provides helpful insights for enhancing health and wellness. This study focused on developing and evaluating a tool to measure individual understanding, perspectives, and habits related to a lifestyle guided by the principles of Persian Medicine. Materials and Methods: This descriptive cross-sectional study included 385 participants who visited comprehensive health centers, selected through convenience sampling. The questionnaire was initially developed using trusted sources in Persian Medicine and refined with input from experts. It consisted of three main sections: awareness, attitude, and behavior. To check content validity, we used CVI and CVR scores, while convergent and discriminant validity were assessed via structural equation modeling (PLS-SEM). We also measured the reliability of the questionnaire using Cronbach’s alpha and composite reliability. Results: After analyzing the data, the final questionnaire consisted of 17 items in the awareness section, 15 items in the attitude section, and 21 items in the behavior section. The Cronbach's alpha values for all three sections were above 0.7, indicating good internal consistency. Additionally, the AVE, composite reliability, and factor loadings were all within acceptable limits, confirming the validity of the constructs. Conclusion: The tool developed is reliable and effective, making it a valuable resource for assessing attitudes and behaviors toward the lifestyle associated with Persian Medicine in health promotion initiatives.
